I found what appears to be a bug. When adding a record that has a file field, it has no problem uploading the file. But when you try to add a file from the modify_form, it ends up using the local path instead of the server path. For example, I just modified a record, and when I view it the name of the uploaded file appears as:

C:\Documents and Settings\PATH\TO\LOCAL\DIRECTORY\pic.jpg

When you try to view it, you get the following error:
